Complete Your Application
Welcome to the Bahama Volunteer Fire Department. We appreciate your interest in joining our organization. The Fire service is an exciting and rewarding profession that we hope you will enjoy. There are a few documents, however, that you must provide us with and complete for your membership requirements.
Please note you will need to complete every section of the application in full as well as upload the following documents to successfully complete the application.
- Have a doctor complete and sign the General Medical Statement Form. You can download this file Here.
- Complete, sign, and date the VFIS Beneficiary Designation form. You can download this form Here.
- A copy of your most current driver's license.
- A copy of your social security card.
- A certified true copy of your driving record. (Obtainable at www.ncdot.gov)
- A certified copy of your criminal history (less than 60 days old), which is obtainable at the magistrate's office.
- If you have not resided in NC for the past 5 consecutive years then you must be fingerprinted at the Durham County Sheriff's Office. (Information will be sent to you upon completing the application.)
- A signed and dated Bloodborne Pathogens form. (Signature collected as part of the application.)
- A signed and dated Hepatitis Vaccination form. (Signature collected as part of the application.)
- Copies of any fire and EMS certificates you currently hold.
Application
Once you have submitted all the above requirements you will have completed the application process. A committee will then review your application packet and will notify you of your membership status. While completing these requirements, you may begin attending training drills at the department. You must attend at least four training drills to receive your turnout gear and fire pager.
